Tips for using evidence sheets

 

  • Always refer to your ETITO Workplace Logbook to ensure that you're completing all the different types of work required.

 

  • Continue to keep a record of the work you do through the use of a diary or similar.

 

  • When you get to the stage where you believe you're competent in a specific task, fill out the evidence sheet/s and get your supervisor to sign it to verify that you carried out the work.

 

  • Some evidence sheets need to be completed multiple times for different jobs [as indicated on the evidence sheet].

 

  • Complete the evidence sheet as soon as possible after you've done the relevant work.

 

  • Once you've completed the required evidence sheet/s for the unit standard and are ready for assessment, the final assessment can be carried out by a Workplace Logbook Assessor. [You must take all relevant evidence sheets and any additional evidence with them to the assessment.]

 

  • If additional evidence, such as a dummy COC or a test report is required, it will be mentioned on the evidence sheet, so read the evidence sheet carefully.

 

  • After you've been assessed as competent, you must send the completed evidence sheets, along with the blue report form from the Workplace Logbook, to ETITO. You must make and keep a copy of the evidence sheets before they are sent. These should be sent to: ETITO, Freepost 5164, Auckland
