Evidence sheets
As an apprentice you need to be keeping thorough evidence of your workplace experience for assessment and moderation. Evidence sheets make collecting this evidence easy - so make sure you're using them!
Evidence sheets - the steps to success
- Always refer to your ETITO Workplace Logbook to ensure you are completing all the different types of work required.
- When you get to the stage you believe you're competent in a specific task, print out the evidence sheet for the associated unit standard.
- Most evidence sheets need to be completed a number of times, so you will have to repeat this process more than once. All evidence sheets must be signed by your supervisor.
- Once you've completed the required number of evidence sheets for the unit standard and are ready for assessment, your final assessment can be completed by your ETITO Workplace Logbook Assessor. [You must take all relevant evidence sheets and any additional evidence with you to the assessment.]
- When you've been assessed as competent you must send the completed evidence sheets, along with your blue report form from your Workplace Logbook, to ETITO:
